Guide to completing your application

This application form is for English Language Studies, Foundation Year Programme, and Homestay Accommodation.

You need to complete only the sections relevant to you. However, some fields are required for all applications.

We have provided further explanations and links to relevant information should you need them.

You are required to provide copies of certain documents to complete your application. These differ according to the course you are applying for. You may also include a cover note if you wish but this is not mandatory

  • Evidence of your English proficiency level e.g. IELTS, Cambridge, TOEFL
  • Letter of Offer for a degree programme at the University of Otago
Foundation Year Programme
  • A copy of your passport
  • A copy of your high school transcripts (all years) or National Student Number (NSN) if you studied or are studying your secondary schooling in NZ
  • Evidence of your English proficiency level e.g. IELTS, Cambridge, TOEFL

  • purposes external to the University when the information is in statistical form or when the University reasonably believes it is not to the student’s disadvantage for such external use to occur, and also where disclosure is required or permitted in accordance with the provisions of the Privacy Act 1993.

I authorise University of Otago Foundation Studies Ltd to release to the student, parents, legal guardian and/or scholarship provider of the student any information about the student which it creates or receives in the course of the student’s enrolment with University of Otago Foundation Studies Ltd, including course results and personal information relating to the student’s well-being.
